Your first visit to Encompass includes some paperwork. These online forms will help you to quickly and efficiently get you in the door and get start with your care. 

After submitting the online forms, please print, sign, and bring the following forms with you to your first visit. Also, bring along any medical information as well including; health insurance cards (dental or major medical) and a list of any current medications.

After your check-in paperwork is complete, you will undergo a comprehensive evaluation.

At that point, we will discuss a treatment plan for future visits, as well as talk about any problems the dentist feels should be addressed immediately or in the near future, such as cavities or periodontal disease.

Plan to be at our office for about 90 minutes, as we will take our time and allow you to ask questions!

  • Did Dr. Frederick Thurston Retire?
    No. He is still the primary provider at Encompass Dental. Only the business name has changed.
  • Is Thurston Dental now Encompass Dental and Wellness?
    Yes! We've just rebranded our look to fit a modern era. Everything you've know and come to expect from Thurston Dental remains the same, just with a different look and feel.
  • When is Encompass Open?
    Our office hours are Monday through Thursday from 8:00 AM - 4:00 PM. However, we know that sometimes dental emergencies happen. Feel free to contact us any time you require our services. If it is after hours, our answering service will let us know you called, and one of our team members will get back to you as soon as possible.
  • What is Compound Procedural Experience?
    Compound Procedural Experience is a calculation that refers to how much experience your provider has in procedural experiences, not years of service. Procedural Experience far outweighs years of services due to compounding daily experiences. For example, as a Painter, the average time it takes to paint one house is 4 - 6 days. Working every day per month with time off would equal 3 - 4 "procedures" a month or between 36 - 48 "procedures" per year. As a Dentist, the average amount of procedures completed per day is between 12 - 18. Working 4 days per week would equal 192 - 288 "procedures" a month or 2,304 - 3,456 "procedures" per year!
  • How much experience does Encompass have?
    Our dental staff has over 48 years of combined experience with many members of the Thurston Comprehensive Dental Center team have been working in this field for more than 20 years! This combined experience creates an advanced, customer-centric experience for every patient.
  • What are Encompass Membership Plans?
    Unlike dental insurance our membership plans have NO deductible, NO allowed amounts, NO limits, NO preauthorization, NO waiting period, NO hidden fees and NO yearly maximums.
  • What is Dental and Wellness?
    At Encompass we work beyond just teeth. We believe that the mouth is just the first step to the rest of the bodes total wellness. Dental and Wellness refers to our services that branch our beyond traditional dentist and include services areas such as; Laser Dentistry, Sleep Apnea treatments, BOTOX, Lip Enhancements, and more.
  • How do I know I will be taken care of?
    You're not just a number. You're family. Encompass Dental and Wellness offers high level of personalized care to patients and their families and allow plenty of time to address any of your personal concerns or issues without feeling rushed. We strive toward knowing you and creating a you personalized care plan. We provide a warm and friendly atmosphere in which our patients are treated out of care and concern. Learning from and getting to know our patients is key in easing any fear or anxiety that you may have during their experience.
  • How do I become a New Patient?
    Becoming a new patient at Encompass Dental and Wellness starts with simple new patient paper work. To expedite your first visit, you can fill this out online here. If you need to fill out your paperwork in office, our professional team can help guide you through to fill out the forms quickly and efficiently. At your first visit, please bring any neccesary medical information with you, including a list of medications you currently take. If you have any health insurance cards (dental or major medical), carry them too.
